How to submit a referral

You can submit referrals to our specialty outpatient services by:

  • Smart Referrals - preferred option
  • Secure Web Transfer
  • fax.

If you can't use Smart Referrals, you can use one of the following templates.


Smart Referrals

We prefer to get electronic referrals through Smart Referrals.

Smart Referrals integrates with your practice management system so it's easy for GPs to track and follow up referrals and mark them urgent if needed.

It also has a directory of public specialist services and helps by auto populating referral information and flagging anything that needs to be attached.

Clinical Prioritisation Criteria

Clinical Prioritisation Criteria (CPC) are clinical decision support tools. They help ensure referrals to public specialist outpatient services are triaged according to how urgent each case is.

The criteria ensures:

  • referrals have all the information they need for triage
  • appointments are given in order of clinical urgency
  • patients are ready for care at their first appointment
  • improved referral and communication processes between referrers and specialist outpatient services.

HealthPathways

HealthPathways is a hospital referral guideline that includes the Clinical Prioritisation Criteria.

It includes evidence-based information on the assessment and management of common clinical conditions.

GPs should use HealthPathways at the point of care. It can also be used by hospital specialists, nurses, allied health and other health professionals in the West Moreton area.

Referring a patient to an emergency department

If you're a GP, you don’t need to call the emergency department unless you need clinical advice.

Make sure your patient has their referral letter with them. You can fax more information to the emergency department if needed.

If your patient is arriving by ambulance, make sure they have their referral letter and any clinical handover documents with them.

Communicating with Queensland Health

Keep your practice and provider details up to date in the Secure Transfer Service (STS) address book. Public hospitals use it to send information, like discharge summaries and clinical letters to GPs.

Accessing your patient's Queensland Health information

If you haven’t already, you’ll need to register for access to the Health Provider Portal (HPP). By registering, you'll get access to a wide range of patient hospital information in one place. It’s easy to use, secure and available whenever you need it.
