Health Provider Portal

You can access your patient's health records through the Health Provider Portal (HPP) website. If you don't have access, you can register on the Queensland Health website.

Records include:

  • pathology reports
  • discharge summaries
  • medical imaging reports
  • medicine lists
  • outpatient letters
  • operation reports
  • test results.

You may also be able to access patient records if they have a My Health Record. Learn more about My Health Record on the Australian Digital Health Agency's website.

Health Information Management Service

You can ask us to send you specific information in your patient's health records. For example, a copy of their colonoscopy report in 2018. If you request multiple records, we may ask you to reduce the scope of your request.

You should get written consent from your patient to access their records. You’ll also need to include your patient's:

  • name
  • date of birth
  • contact details.

You can send your request to the hospital or facility where your patient was treated, using the contact information below.
